FOR NEW PATIENTS
It is important to fill out any and all medical intake forms and return it to our office at least 7 days prior to your appointment. You can return it to our office by utilizing your patient portal, mail, or fax.
Having these forms 7 days in advance will allow Melissa Santiago, ND or Gwieneverea Brandon, MD to help solve your problems more efficiently and enhance the quality of your care. If your Intake Form and Medical Records have not been received at least 7 days prior to your initial appointment, it may take Melissa Santiago, ND or Gwieneverea Brandon, MD up to 30 minutes of your appointment time to review your chart.

Your initial visit will include a 90-minute consultation with Melissa Santiago, ND and/or Gwieneverea Brandon, MD. Nutritional therapy and laboratory/diagnostic testing are integral components of your treatment plan. Test results are used to design your personal health care program as well as uncover the root causes of your medical condition. Nutritional supplements are often recommended, and we will help you select and find the highest quality products.

*PLEASE NOTE IF YOU DECIDE AFTER YOUR LIMITED CONSULTATION THAT YOU WOULD LIKE TO UPGRADE TO A FULL PATIENT, YOU WILL BE CHARGED THE FULL INITIAL PATIENT CONSULTATION FEE OF $525 ASIDE FROM THE LIMITED YOU ALREADY PAID.
You will be charged your entire appointment fee for appointments missed or canceled with less than 24 hours’ notice. You will be charged a cancellation/rescheduling fee of $75 if you contact our office at least 48 hours before your appointment. Only 1 reschedule will be allowed and special consideration may be made for circumstances beyond your control.
Payment for all charges is due at the time of service.
Medical records can only be released with your authorization. It is your responsibility to obtain previous medical records from other physicians, or health care providers that you wish Melissa Santiago, ND or Gwieneverea Brandon, MD to review. Please contact your physician or other health care provider to obtain these records and make sure that we have received them at least 7 days prior to your initial appointment.

Due to HIPAA and privacy practices, you must call for any medical related questions or send a message through your secure patient portal.
Our office accepts cash, checks, credit cards (MasterCard, Visa, Discover) and Care Credit for services rendered. When you schedule your initial visit, we request new patient consultation payment up front. On the day of your scheduled appointment, all charges for laboratory testing and nutritional supplements will be itemized, and payment is due on the day of service.
Follow-up phone, or in person consultations will be billed to your credit card on file unless you provide other payment information and instructions prior to your appointment. If additional lab tests are required and our office sends test kits, the appropriate fees will be charged to your account. Credit card on file will also be used for supplements mailed unless otherwise specified.
Opus Wellness Center DOES NOT ACCEPT INSURANCE and we cannot assist you with claim resolution. In addition, we are not Medicare providers. You will be provided with a billing summary which you can submit to your insurance carrier.
Our office hours are Monday, Tuesday and Thursday 9:00 am -6:00pm, Wednesday 9:00 am – 5:00 pm, Friday 10:00 am to 2:00 pm CST.
If you are going to stop by the office to pick up supplements, we ask that you kindly email your order to us at email@example.com or call/text 512-593-8443 prior to your visit (to ensure items are in stock).

IV FUSION ROOM
We have an open IV Fusion Room that will be shared by multiple patients. Please adhere to the following:
